    • Observed table tops and bottom of equipment to have an accumulation of pretzel salt. 

Deer Creek Marina

  • Non-critical: 3717-1-02.4(A)(2) / PIC – Level Two Certified Manager
    • Facility does not have at least one employee with manager course certification in food protection. However, facility no longer sells and/or prepared pizza and will be bumped down to a risk II next licensing year where this certification is not required.
  • Non-critical: 3717-1-04.5(C) / Cooking and baking equipment – cleaning frequency
    • Observed inside of microwave for customers to heat up prepackaged sandwiches to have food debris and old splatters. Please clean at a frequency necessary to prevent this buildup and splatter.
  • Non-critical: 3717-1-05.1(S) / Plumbing system – maintained in good repair.
    • Repeat Hot water tank is still not fixed and no hot water is available within the facility. Handwash sink in kitchen area does not work and leaks everywhere when hooked up to water. Please ensure that the water tank and plumbing for the hot water and handwashing sink is fixed this year so that hot water can accesible for handwashing and warewashing of utensils.

  • Critical: 901:3-4-15(B) / Embargoing of food due to adulteration, misbranding, or expired baby formula or baby foods
    • Corrected During Inspection Observed twelve (12) 12.9oz Gerber Soy instant formulas on the shelves that all had expired dates including January 11 2019, April 13 2019, and June 1 2019. Manager pulled the items from the shelf and placed them in a box in her office and labelled them to be returned to vendor for credit. Corrected.
  • Non-critical: 3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
    • Observed crumbs and food items (pre-cooked chicekn breast and pizza dough) on the ground underneath the shelving units in the walk-in freezer.
  • Non-critical: 3717-1-06.4(B) / Cleaning – frequency and restrictions.
    • Observed crumbs, old food, and old food splatters over the cabinet fronts, inside the drawers, and on the shelving units throughout the kitchen. Ensure that these areas are cleaned at a frequency necessary to prevent these accumulation of crumbs, food, and splatter.

  • Non-critical: 3717-1-06.2(E) / Handwashing signage.
    • Observed no handwashing sign reminding employees to wash their hands before returning to work in either employee restroom.
  • Non-critical: 3717-1-06.4(A) / Repairing.
    • Observed water damaged ceiling tiles in the back storage areas and broken/missing ceiling tiles along in the employee area.

  • Non-critical: 3717-1-04.5(B)(5) / Equipment food-contact surfaces and utensils – cleaning frequency.
    • Observed some debris in the ice chute from the soda machine. Clean to prevent anything from falling into drinks during prep.
  • Non-critical: 3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
    • Observed some ice build up in the walk in freezer on a box under the condenser. Maintain equipment in proper repair to prevent this build up.
  • Non-critical: 3717-1-06.4(B) / Cleaning – frequency and restrictions.
    • Observed that the facility had some duct tape holding the base cove to the wall in the office. Properly seal to prevent dirt from being caught behind the molding. Observed the kick plates on the walk in cooler/freezer door were collecting food. Clean and caulk to prevent food from becoming stuck in these areas.

Subway

  • Critical: 3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food – cold holding.
    • Observed sliced tomatoes and teriyaki chicken at 48F and 49F respectively. During the inspection the other items on the cold line ere holding the proper temperature and the employee did not know believe that the foods had been out of the cooler for prep. The cooler temperatures stated 44 and 51F during the inspection. Maintain all cold foods at the proper temperature of 41F or below to prevent temperature abuse.
  • Critical: 3717-1-07(B) / Poisonous or toxic materials: Working containers – common name.
    • Corrected During Inspection Observed a chemical container that was not properly labeled. During the inspection the PIC labeled the container. Properly label all chemicals to prevent misuse.
  • Non-critical: 3717-1-02.3(A) / Food contamination prevention – eating, drinking, or using tobacco.
    • Observed employees were eating food on the prep table. Employee foods shall not be consumed in an area where it may contaminate foods. The PIC properly cleaned the prep table during the inspection.
  • Non-critical: 3717-1-04.8(A) / Equipment and utensils – air-drying required.
    • Repeat Observed pans at the facility were stored before fully air drying. Allow utensils to fully air dry prior to stacking.
  • Non-critical: 3717-1-04.4(B) / Cutting surfaces.
    • Repeat Observed that the cutting board on the veggie slicer was becoming very frayed. To prevent plastic from falling into foods that cutting board shall be replaced or otherwise resurfaced.
  • Non-critical: 3717-1-06.1(A) / Floors, walls, and ceilings.
    • Observed that the door sweep entering the kitchen had dirt around it and was loose. Maintain surfaces smooth and cleanable.

Speedway #3737

  • Non-critical: 3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food – cold holding.
    • Repeat Per PIC he has the servsafe class through speedway, but does not have a copy of his certificate. Provide a state approval certificate for the food managers class.
  • Non-critical: 3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
    • Repeat Observed that the beer cooler door seal was damaged. Repair. Observed that the cappuccino machine door latches were barely functional. Maintain equipment in good repair.
  • Non-critical: 3717-1-05.4(P) / Maintaining refuse areas and enclosures.
    • Repeat Observed boxes and trash around the dumpster enclosure. Clean around dumpsters to prevent attracting pests.
  • Non-critical: 3717-1-06.4(A) / Repairing.
    • Observed some cove molding peeling off the wall in the rear of the facility. Reseal the cove molding to the wall to prevent any damage during mopping.
  • Non-critical: 3717-1-06.4(B) / Cleaning – frequency and restrictions.
    • Repeat Observed dust underneath the shelves, observed wrappers and other dry debris in the walk in cooler and the in the dry storeroom. Dry clean these areas thoroughly and often to remove dust, wrappers, and other debris.
