  • Non-critical: 3717-1-02.4(A)(2) / PIC – Level Two Certified Manager
    • Repeat Facility does not have an employee with level two certification in food protection. The manager has servsafe, but they do not have the ODH level 2 certificate. The course you take must be approved by ODH and a certificate obtained.
  • Non-critical: 3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
    • Equipment components are not intact, tight or properly adjusted. The gasket on the Walk-in-cooler needs to be replaced.
  • Non-critical: 3717-1-05.1(S) / Plumbing system – maintained in good repair.
    • Plumbing system not properly maintained or repaired. The mop sink has been removed. They said it is being replaced with a new mop sink. Correct By: 26-Jun-2019
  • Non-critical: 3717-1-05.4(F)(1) / Outside receptacles – tight fitting lids
    • Corrected During Inspection Outdoor refuse containers without tight fitting lids, doors, or covers. Lid open on dumpster.
  • Non-critical: 3717-1-06.4(A) / Repairing.
    • Repeat Physical facilities not maintained in good repair. Repair broken tiles, and the strip on the wall by the hood.
  • Non-critical: 3717-1-06.2(E) / Handwashing signage.
    • Observed men’s bathroom to not have an ‘Employees Must Wash Hands’ sign posted at the handwash sink.
  • Non-critical: 3717-1-06.1(M) / Outer openings – protected.
    • Repeat Observed back door near the dry storage area to be in poor repair and exposing outer openings to the facility.
  • Non-critical: 3717-1-06.4(A) / Repairing.
    • Repeat Observed physical facility to be in poor repair, including but not limited to water damaged ceiling tiles, broken floor tiles (kitchen area, bathrooms, walkin cooler, dining area), missing and/or broken floor coving, missing wall tiles in women’s restroom.
  • Non-critical: 3717-1-06.1(I) / Light bulbs – protective shielding.
    • Observed light fixtures with out a shield resulting in exposed light bulbs in the hallway near the employee area and above the breading station.
